That suspension will prevent the coach from taking part in the Buckeyes' preparations for the season opener against the Beavers Sept. 1.
The report states Meyer was aware of the coaches' trip to the strip club.
OSU also announced that athletic director Gene Smith has been suspended just over two weeks from August 31 to September 16. His suspension will be without pay as well.
On Wednesday, Ohio State announced that it was suspending head football coach Urban Meyer for three games after an independent investigation into what he knew about the alleged serial abuse committed by former assistant coach Zach Smith against his ex-wife Courtney.
Meyer has said he handled accusations properly when he found out about them, but acknowledged he lied to reporters at first when he said he hadn't heard of the 2015 incident.
When you think about it, if Meyer has memory issues, does that make him unqualified to be a head coach?
"Coach Meyer and AD Gene Smith, although acting in good faith, did not report the (police) investigation of Zach Smith.to (the Title IX) compliance (department) as we believe they should have", said Mary Jo White, who headed the investigation.
Meyer admitted his loyalty to his mentor Earle Bruce, who was Zach Smith's grandfather, probably made him too lenient with Smith. Meyer later corrected his statements, after being placed on administrative leave, saying he knew about the accusations and reported them properly.
"However, I should have demanded more from him and recognized red flags".
The decision came down following a two-week investigation into Meyer's handling of domestic violence claims against former Smith, the team's former wide receivers coach.
Let's be honest, shall we: If Meyer were a lesser coach, he would have been fired on the spot, but because he is one of the greatest head coaches of the modern era, he was allowed to keep his job by Ohio State's kangaroo court and its dog- and-pony-show investigation.
Ohio State President Michael V. Drake will have the final say on whether Meyer is fired or faces other consequences.
At the news conference following the exchanges, Meyer categorically denied having any knowledge of any allegations against Smith in 2015. Zach Smith has never been criminally charged with domestic violence. As a result of that, OSU is expected to revamp their hiring procedures and policies - and background checks - when it comes to prospective coaches. It came out in the investigation that people on that staff knew that Zach Smith, the assistant coach who was sacked, they knew that he was having sex with a staffer, but they never told Urban Meyer. Do you have a restraining order? "I got investigated twice, and no charges ever came about it because I didn't do anything physically to harm her".
The assistant coach and his ex-wife have said both Gene Smith and Meyer's wife, Shelley Meyer, an instructor in Ohio State's nursing school, knew of a 2015 instance of alleged domestic violence. Its late-night release precluded reporters from asking Meyer about many of its detailed findings when he was available. "And for that, I am deeply sorry", Meyer said. Meyer said in his tweet that he always elevated issues through the proper channels, and did so with the Smith situation in 2015.
While Meyer offered an apology to "Buckeye nation", he drew the ire of critics for failing to properly apologize to Courtney Smith.
Gene Smith said Wednesday night, "I fully support the findings of the report, and the subsequent actions that the university has taken".
